There are two causes for this. This menu loads the most recent 6 tasks completed by the player, but is only capable of displaying the oldest 3 of these tasks, so the information will often appear to be outdated. Secondly, while Rare had written summaries and clues for approximately half of the game's tasks, they had only assigned gameplay flags to a small handful of these text strings as of December 2000 (presumably leaving it for a later stage in development when gameplay flags etc. would've been more finalised) - as a result, this screen (and the pause menu's % counter) will stop updating early on in the game.
